It defines electronic health records as: ‘digital tools that help people to maintain their health and manage their care by enabling them to capture their own health and care data, to communicate with health and care services, or to have access to their care record.’

Users, suppliers and implementers of personal electronic health and social care records are invited to fill in the survey online, until 13 September 2015.
